Tournament Rules and Forms

Tournament Entry Process:

  1. Print and mail entry form below, include your check, this is the only way to reserve a spot.
  2. Teams must register online for USSSA registration number ($30) at www.usssa.com. JP Sports will NOT register your team. The registration number should be included on the tournament entry form, but can be submitted at a later date prior to the start of the tournament.
  3. The JP Sports and Events Management Team Roster and Waiver form below is required at check-in forty five minutes before your first game. Print roster/waiver form, it must be signed by a parent for each player and presented to our site-supervisor at check-in.
  4. USSSA requires all teams register your team roster online at www.usssa.com. This is required for all USSSA tournaments. Your team will not be allowed to start tournament play until this is completed. Print a copy of the roster and give to our site-supervisor at check-in. No parent signature is required on this roster.

Once entries are received an a verification email will be sent, and teams will be listed on the Who's Coming under the Tournament Information section.
